Hospital, Rehabilitation and Recovery Care

When an elderly patient is hospitalized for a medical procedure or illness stress and confusion often occurs.  Recovery can be lengthy process with a set of unique challenges. A Department of Health & Human Services study found that in April 2015 close to 20% of Medicare recipients with acute conditions (heart issues, pneumonia and major joint replacements) were readmitted to the hospital within 30 days while close to 14% of Medicare recipients with all other conditions were readmitted within this time frame.

Reduce the chances of Hospital Readmission with Post Hospital and/or Recovery Care

Hospital readmissions have been linked to the absence of outpatient follow-up care, inadequate understanding of discharge instructions and the scarcity of help for those transitioning from hospital to home (US National Library of Medicine, National Institute of Health). Ray of Sunshine Senior Care and professional caregivers can help reduce the chances of being readmitted by providing supportive services after hospitalization and during recovery.

A professional caregiver can be available before discharge to ensure a smooth transition and accompany the client home. If requested, we can also coordinate services with home health or physical therapy agencies involved in the recovery care.

Professional caregiving referrals are available on an hourly or live-in basis. If a client lives alone they often start out with live-in care and transition to hourly care as their recovery progresses. Referrals are frequently made to assist with:

  • Monitoring of medication schedules and any dietary guidelines or restrictions

  • Escort to follow up appointments and attend as requested

  • Additional updates to family and loved ones

  • Companionship and support

  • Meal preparation and grocery shopping

  • Light housekeeping including laundry and changing linens

  • Assistance with bathing, grooming & dressing

  • Mobility assistance & support

  • Assistance with errands

  • Social events and activities

  • Coordination of care with home health and hospice

  • Other customized services available based on your unique needs
